Full-Time Clinical Pharmacist

Job Description

We are seeking a qualified Clinical Pharmacist to review patient drug profiles to make sound clinical recommendations to the medical staff of the hospital (under the general supervision of the Director of Pharmacy).  The job location is south of Pittsburgh, PA.

Clinical Pharmacist Education and Experience:

  • Valid degree from an ACPE accredited school of pharmacy
  • Valid Registered Pharmacist Pennsylvania state license
  • Post Graduate Residency is required.

Clinical Pharmacist Salary / Benefits:

  • The salary range is $100K-$110K+, based on experience – plus a comprehensive benefit plan.


Clinical Pharmacist Principle Accountabilities:

  • Work in conjunction with Infectious Disease on the implementation of an Antibiotic Stewardship Program and attend the Infection Control Committee meeting that occurs quarterly.
  • Maintain and monitor the Intravenous to Oral Conversion Program based on various criteria for the step-down program to reduce costs and intravenous-related adverse events.
  • Maintain and monitor the Renal Dosage Adjustment Program by working with the Medical Staff to keep up to date information based on medications that need adjusted for renal insufficiency.
  • Creation of Drug Monographs to be reviewed with nursing and presented at the Pharmacy and Therapeutics Committee meeting that occurs once every two months for review for a closed formulary system.
  • Work in conjunction with the Director of Pharmacy and Risk Management to review and answer Medication Errors on an as needed basis and attend the Medication Errors Task Force meetings that occur monthly.
  • Attend the quarterly interdisciplinary meeting that focuses on Pain Management.
  • Attend the Cardiology Committee meeting as the Pharmacy representative.
  • Attend the Critical Care Committee meeting as the Pharmacy representative that occurs every quarter.
  • Main administrator for the Hospira Smart Pumps for programming and updating the drug information library.
  • Oversee all Highmark Care Initiatives, such as VTE prophylaxis, first dose of antibiotics within 6 hours of admission for all pneumonia patients, completion of prophylactic antibiotics within 24 hours of the first dose of antibiotics for surgery patients, etc.
